A review of the sixteen treatment plan reviews that were done found that all contained required information, including youth progress made toward goal completion, changes in diagnoses, changes in interventions used, changes in target dates, and a summary of the treatment plan review. It was also found that all sixteen treatments plan reviews done contained signatures of the youth and the licensed mental health staff, which also completed the treatment, plans review. Of the nine files youth files reviewed, two were closed files from youth who were no longer at the program, and were applicable for requiring a mental health substance abuse treatment discharge summary to be completed.
