In the crystal structure, glutamic acid 104 is 10 A from the active site serine-70 hydroxyl, involved in the acylation reaction which is the first step in , -lactam hydrolysis. It is 6.9 A from the carboxylate of glutamic acid 166 involved in the deacylation step ; and 10.4A from lysine 73 which, according to Strynadka et al. 1992 ; , acts as the general base in the acylation step. Patients for DXA are routinely advised to avoid calcium supplements on the day of their examination in order to avoid tablet artifact. Poor tablet dispersion may result in passage of the intact tablet distal to the stomach where it may overly the vertebral bodies. The tablet was identified in the 8.5 minute scan overlying L3. On both the 10 and 12 minute scans, the tablet was to the left side of the abdomen, not overlying vertebral bodies. Without artifact, L1-L4 BMD 0.976 gm cm2, BMC 64.79 gm, T -0.6; with the artifact, L1-L4 BMD 0.990 gm cm2, BMC 66.44 gm, T -0.5 a 1.43% increase in BMD. Without artifact, L3 BMD 1.021 gm cm2, BMC 17.68 gm, T -0.6; with the artifact, L3 BMD 1.080 gm cm2, BMC 19.06 gm, T 0.0, a 5.77% increase in BMD. Software deletion of the overlying tablet area resulted in a return to L3 BMD equal to baseline. We conclude that undissolved calcium tablets present a significant artifact on individual vertebral analysis but minimally impact L1-L4 BMD. The relative impact would be greater in patients with very low bone density.

Finally, a very recent investigation showed that clodronate significantly inhibited both human PMN MMP-8 ; and GCF collagenase activities as assayed by Type I collagen degradation ; in the 750-1000-jxM range in vitro. Fifty percent inhibition of the activity IC50 ; of pure human PMN MMP-8 was achieved at 150 JLM clodronate Teronen et aL, 1997 ; . In contrast, dixycycline had been shown to inhibit the activity IC50 ; of human MMP-8 in the 10-30-|iM range Sorsa et aL, 1994 ; . These far greater concentrations of BPs that are needed to inhibit extracellular MMP activity further reinforce the concept that BPs act primarily through an inhibition of osteoclast function, whereas tetracyclines and their chemically modified analogs CMTs ; , as inhibitors of bone resorption, affect not only MMP activity but also osteoclast development, function, and structure. Future investigations should elucidate further the mechanisms by which TCs CMTs modulate bone metabolism.

METHODS: Ten patients were selected with a slowly progressive, asymmetric, lower motor neuron disorder, and were treated prospectively with IVIg at a dose of 2g kg over 5 days. All subjects had neurophysiological testing to look for evidence of conduction block before treatment. Muscle strength was assessed by MRC grades and hand held myometry, measuring pinch and grip strength. A 20% increase in both pinch and grip myometry was considered a positive response. RESULTS: In no patient was conduction block detected. Four of the 10 patients showed a positive response to IVIg, with the best response occurring in two patients who presented with weakness but without severe muscle wasting. Three of the four responders have continued to receive IVIg for a mean period of 17 months range 15-24 months ; , with continued effect. The response to IVIg was not related to the presence of anti-GM1 antiganglioside antibodies, but responders had a selective pattern of muscle weakness and normal 90% predicted ; vital capacity. CONCLUSION: The findings suggest that a course of IVIg should be considered in patients with the clinical phenotype of multifocal motor neuropathy but without neurophysiological evidence of conduction block.
