However, they often find themselves in the predicament of wanting to treat seemingly legitimate patients without having information about their patients' prescription drug and medical histories that would help them identify and address any problems. Indeed, a 1999 report from the Institute of Medicine stressed that most medical errors do not result from individual practitioners' recklessness; rather, they are attributable to faulty processes and systems that lead people to make mistakes or fail to prevent them through lack of information and support in a complex working environment.20 Solving problems within healthcare requires the design of systems and processes to help, for example, inhaler. Bricanyl felineStanford University, Palo Alto, CA, USA As we gather in Lyon this September 2003 to celebrate the career of our esteemed colleague, Michel Jouvet, basic sleep research and sleep medicine are fully established and recognized as an independent scientific, clinical discipline. Basic sleep research is being vigorously pursued throughout the world, and sleep disorders clinics can be found in almost every nation. It is likely that paradoxical sleep would have come under intense scientific scrutiny sooner or later, but without the efforts of a few outstanding pioneers, maybe not. As has been pointed out many times, the observation of the occurrence of rapid eye movements during sleep by Gene Aserinsky and Nathaniel Kleitman should not be glibly identified as the discovery of REM or paradoxical sleep. The lineage of incidental observations on EEG wave forms during sleep and all night sleep patterns is clear, but the development of sleep research as a respected scientific discipline can be credited almost entirely to Michel Jouvet. To play such an important role requires being in the right place at the right time, but added to this must be a very outstanding scientific ability, a unique propensity for leadership, and a great and clear vision of the future with the drive to make it happen. Finally, and also very important is the ability to reach out to other scientific disciplines and incorporate new methodologies into the research. The most notable example involving Michel Jouvet is the application of histofluorescence techniques and the study of monoamine neuroanatomy and neurochemistry and the identification of these systems in the regulation of sleep states. How much of a role chance plays in bringing key individuals to the fore can always be debated. In my own case, I went to college with the intent of pursuing a career in journalism, but I was late and the classes were full. As a consequence, I made a grudging second choice to enroll in a psychology class. The class happened to be taught by a psychoanalyst and it was there that I met Dr. Sigmund Freud for the first time. My own intense interest in rapid eye movements and dreaming would not have existed had I not become a passionate, though nave disciple of Dr. Freud. I believed without criticism that the study of dreaming sleep would provide the royal road to the understanding of mental illness. For his part, Michel Jouvet might not have survived fighting in the resistance during WWII, and I'm sure that some twist of fate intervened to bring him to Riverside, California to work with Horace Magoun and colleagues. At the time, the most important paradigm in neurophysiology was the concept of the reticular activing system and its control of wakefulness, EEG activation, and consciousness. I know that Michel has pursued more than one path down a blind alley as did I, and possibly a little additional luck or insight would have led somewhere else. But, the stage must be set.
